Health care practitioners: "Each kit will come with a requisition form to select available tests.  Practitioners are not charged for kits. Practitioners may call and set up an account in order to have kits drop-shipped to patients."

Patients: You may order a shipping and collection kit directly from Spiro Stat that you can take to your health care practitioner.  You will initially be charged $50 for the kit cost and shipping fees.  You will receive a $50 credit toward your testing service fee of $500. However, please note, only licensed health care practitioners may collect samples and sign test requisition forms.  Please ensure that forms are filled out completely as incomplete requisition forms will not be processed. Results are returned to the ordering practitioner usually within 7 to 10 business days.
